Basically the President then nominates a successor that has to be approved by Congress. You can find details of this in the 25th Amendment to the Constitution.

"Section 2 of the 25th Amendment provides that "Whenever there is a vacancy in the office of the Vice President, the President shall nominate a Vice President who shall take office upon confirmation by a majority vote of both Houses of Congress." Gerald Ford was the first Vice President selected by this method, after the resignation of Vice President Spiro Agnew in 1973; after succeeding to the Presidency, Ford nominated Nelson Rockefeller as vice president."

The president would appoint a new Vice President, whom the legislative branch would ratify. In the meantime, if something happened to the President, I believe the speaker of the House (currently Nancy Pelosi) would be the next person in line.

I think it's the Speaker of the House. There is definately a chain of command beyond the VP and it is followed.

The senate and congress will elect one for us after the Prez names his choice, it must be approved by the congress. Go to the constitution and read.
