I think it could be a symptom of a disorder, rather than a disorder in its own right. But how do you define the boundaries of self mutilation? For example, are tattoos mutilation or art? So maybe, it's more a person's reason or motivation for mutilation, which brings me back to my original point - that it could be a sign of an underlying illness.
