It is implied that he and Bleeding Gums Murphy are long-lost brothers; Hibbert says he has a long-lost brother who is a jazz musician, and Murphy says he has a brother who is a doctor that chuckles at inappropriate times, but somehow the two don't put these clues together. He also bears a striking resemblance to the director of the Shelbyville orphanage, who mentions a personal quest to find his long-lost twin to an indifferent Homer

Dr Hibbert has a brother in 'Oh Brother Where Out though' when Homer goes to see about his long lost brother at the orphanage and there is a man who looks like Dr Hibbert and he says he has a long lost brother.
His wife Bernice is based on the wife in the Crosby show and so is the rest of his family

Dr Hibbert's wife is Bernice. Bleeding Gums Murphy is his possible long-lost brother, indicated in the episode "Round Springfield":
Bleeding Gums: "I don't really have a family, all I had was a little brother who grew up to become a doctor. He used to laugh at the most inappropriate times."
Dr. Hibbert: "Hey I've got an older brother that I'll never see. He's a jazz musician or some such. Oh well, bye, bye."

Dr Hibberts brother is the person from the adoption agency who tells Homer where his brother is but im afraid i dont know his wifes name. sorry x x
Bleeding Gums Murphy is also meant to be related to them both x x
