There were actually 1,527 MOH certifications during the Civil War. So narrowing it down could be difficult.

I found someone from Maysville, Iowa. Nobody listed for Maysville, Kentucky.....

May, William C
2- private, Company H, 32nd Iowa Infantry
3- Nashville, TN, 12-16-64
4- Maysville, Franklin, IA
5- Pennsylvania
6- 2-24-65
7- Ran ahead of his regiment over the enemy's works and captured from its barrier the flag of Bonanchad's Confederate battery (C.S.A.)

You can narrow the odds by searching for a list of Medal of Honor winners. You can then research those who were awarded it during the Civil War. I believe there were only eight recipients and the Medal of Honor was created at that time.

1522 people received the Medal of Honor for service during the Civil War.

You need to narrow it down just a bit with some more information.
