My question is w.r.t platform styled footwear especially. Do they harm the back/arches etc as much as the conventional heels?

One thing is clear...From the point of view of physiology, the way you walk when wearing high heels is not normal; our feet are designed to walk optimally in a different position than the one they have when walking on high heels...The whole weight-related stress distribution is misplaced; walking on high heels puts stress on places that are not supposed to bear stress and removes stress from the places where stress should really fall; the long-term imposition of such stress to your lower extremities and back can definitely have a negative impact ultimately leading to back pain, misaligned joints (classic is the first metatarsopahlangeal joint - the joint at the bottom of your big toe - which is so frequently affected; actually I have never seen a male with this type of misalignment), arthritic changes, distortion of normal foot anatomy, etc...
You ask about platforms...As long as you wear a shoe that significantly increases the angle defined by the ground and the sole of your foot, the maldistribution of weight along your lower back and lower extremities increases proportionately...

If you wear them alot, it can cause the tendons in the heel to shorten. Which in turn causes tendonitis. It is very painful. It can also cause the area behind your toes (ball of foot) to break down. Also, wearing high heels can make you more succeptable to sprained ankles and falling down.

Not if you do it infreqently, but if you persist in walking like a ballerina, you will develop early arthritis and podiatary imbalance, impacting the streanth of your arches.

hell yeah!!! specially in european countries such as Italy, a lot of woman get back problems for being so fashin addicts...be careful with the shoes you use.... then again wearing flip flops or sneakers will always be the most comfortable option

Heals are bad for one's back, knees, ankles, arches, and hips. They are VERY hard on one's joints....worse even than running.
