The first thing you have to do is to accept that you are lazy and unmotivated. It's good that you don't want to be that way but it's better if you accept yourself first. You are perfectly fine with the fact that you are lazy. That's your character. There is no good or bad, there just is.

So, now you've accepted that you're lazy. Now it's time to do something about it. You need to do a discipline. A simple daily discipline that you will do EVERY DAY. This is the ONLY THING that you do outside of your normal routine. This could be as simple as making the bed every day, or flossing your teeth, or walking the dog - anything.

As you do this day after day after day your character will subtly change. You will notice things about yourself you never noticed. You will discover other things about yourself that you aren't happy with; you'll find things about yourself that you're very happy with. It's a process of discovering YOU.

Then do another discipline - do them for at least 3 months. Start with only one but as you get better you can do more.

Good luck - you are perfect JUST THE WAY YOU ARE.

Instead of pointing you to, say, a self-help book, let me recommend to you a short fiction called What Makes Sammy Run. Sammy wasn't the best person, but he was motivated, and I bet it inspires you.

You motivate yourself by saying "I will treat myself if I do activity x before y o'clock"

You can also consider the good results which will come from doing those tasks such as getting fitter and looking better after exercise.

I find the approach by Stephen Covey in his famous book (see source) to be thorough and effective--IF you really apply yourself to understanding it and working for it. But he is inspiring: just read his bio and you'll see what motivation can do to spread joy in many lives.

Good luck!
