Usually, the insurance company will get the police report they need for the accident. IN Texas, you can go to the DPS and get a copy of the report if you have the location of the accident, name of parties involved, date of the accident. Costs $6 for the copy.

Either call or look online for your state police department information. They can give you the website if you call the local number, not 911. To find quicker do a yahoo web search using your state name and police department. If you where told the form was available online, then you should find a link for forms on the site. Not all police departments offer this online.

You will most likely need to go to the police station that took the report. Theres usually a small fee of 10-20 dollars payable by money order as well. I would call them to find out details.

that stuff can be obtained online but be careful, there a lot of fake sites out there that will scam personal information. Your best option is to go to the state police directly they will give you a copy

Your insurance company should be able to request it if they are doing their job. If not than you call the State Police and explain the situation. They are usually very helpful.
