Depends on the sport. Each have their governing bodies which decide the team.

As a participant you must first qualify for you countries Olympic team in whatever sport you wish to participate in. Then your team or your individual performances must qualify for the Olympics.

Ex. To make the British swim team you would need to place high enough in the British nationals. But if your best time does not meet the Olympic minimum standards you won't make the Olympics. (exception to the second part of this rule is that the host country gets auto entry to all events.)

If its a team sport usually there are regional qualifies to make the Olympics. (This is how the teams are chosen for soccer, basketball etc.)

All qualifying processes are determined by the individual sports international governing body.

Simplified, here is the process:
1. Get to be top athlete/team in your home country
2. Go around the world and compete in various qualifying events. Qualifying is set into regions and world.
3. Place in a qualifying event. The top 3-5 athletes/teams per region and the top 5-8 athletes/teams in the world are allowed to participate in the Olympics.

EXCEPTION: Host country is allowed 1 athlete/team in every division of every sport.

Qualification rules for each of the Olympic sports are set by the International Federation (IF) that governs that sport's international competition.

For individual sports, competitors typically qualify through attaining a certain place in a major international event or on the IF's ranking list. National Olympic Committees may enter a limited number of qualified competitors in each event (3 is a common number), and the NOC decides which qualified competitors to select as representatives in each event if more have attained the benchmark than can be entered. Many events provide for a certain number of wildcard entries, given to athletes from developing nations.

Nations qualify teams for team sports through continental qualifying tournaments, in which each continental association is given a certain number of spots in the Olympic tournament. The host nation is generally given an automatic qualification.

Qualification criterias are set by the country's sports governing body. To qualify to participate on an Olympic Team -

Short version
You would train hard for many years, then go to your country's Olympic trials of your particular event and then the top three in each event at the trials will qualify for an Olympic berth. The top three will also have to meet the Olympic 'A' standard in their event to qualify.

Long Official version

an athlete must have achieved a qualifying mark established by the Sports Governing Body in a national or international meet which is sanctioned or recognized by either Countries sports association and the IAAF and is conducted under the rules of same. Such a qualifying mark must be achieved during a time period prescribed by the Sports Committees.

IAAF Requirements:

The Country is allowed to enter up to 3 athletes per event provided they have all met the IAAF "A" qualifying standard.
The Country may enter only 1 athlete per event if she/he has met the IAAF "B" qualifying standard.

Individual Event Selections
Rank Order of Finish: All athletes competing in an Olympic Trials event will be assigned a rank based upon the order of finish for that event. An athlete must finish1 each event in which she/he is entered (declared) in order to be assigned a rank. Rank order of finish for an individual event will be established as follows:
Running events: 1) Athletes competing in the final round of competition will be assigned a rank based upon their place-finish in the finals: 2) Athletes participating in the semifinal round of competition will be assigned a rank based upon the fastest times achieved in the semifinal round; and 3) Athletes finishing in a preliminary round of competition will be assigned a rank based upon the fastest time achieved in the preliminary rounds. An athlete's final ranking will be based upon the highest round completed by an athlete.
Field events: 1) Athletes participating in the final round of competition will be assigned a rank based upon their place-finish in the finals; 2) Athletes finishing the qualifying rounds of competition will be assigned a rank based upon the athlete's best mark. An athlete's ranking will be based upon the highest round completed by an athlete. If there is a tie in a field event, excluding the high jump and pole vault, the tie will be broken by selecting the athlete with the best second mark. Ties in the high jump and pole vault will be broken by a jump off, to the extent that it is necessary to nominate a team member in that particular event.
Olympic "A" Standard: The number of athletes who have achieved the Olympic "A" standard in a designated event will determine the number of athletes who will be nominated (up to 3) to the Olympic Team for that event.
will nominate to the OC the names of four athletes in each individual event. These athletes are then considered to be Candidates for the Olympic Team. Only these athletes may subsequently be entered to compete and thereby become official Olympic Team members, based upon the qualifying standard met by these individual athletes.
