I'm planning on transferring to another school and change course in college and i need to make a request asking for a honorable dismissal...

I don't think you need that. You need to select your other school, contact them and find out what is needed to apply. Get accepted. Then just notify your current school that you will not be returning.

The school you are transferring to will likely want to see your transcripts. If your achievements haven't been stellar, it might help to write an application letter with an explanation of that and how you expect to better your performance in the future. Might help to have a letter from a professor who knows and supports you as well.

But if poor academics is a problem, you'll either need to stay where you are and pull up your grades for a couple semesters, or transfer to a community college and do the same thing, then make another transfer to the new university. . . . .

I've never heard of "honorable dismissal" letter.
