Two things immediatly popped into my head:
1st - every breast feed? if so, sometimes babies reject formula and bottles in general because they prefer the other
2nd - have you considered if your baby is lactose intolerant?
Lactose intolerant can simply mean, in an adult, that you get gas - your farts stink bad - you need to use the restroom after drinking milk #2 etc... It isn't like the hives etc...

I have a 3 1/2 month old baby and he's a preemie too, @ about a 3 weeks his father and I noticed that he wasn't taking his formula well and would spit up, very frequent. He was diagnosed with pyloric stenosis and a little after 1 1/2 month he needed surgery. Make sure you take your baby to the doctor and request an ultra sound that's the only way to find out because a doctor might just try to change the formula and this is something serious that can cause death. Good luck and I really pray your little angel is okay. By the way my son is doing very well after the surgery he started gaining weight quick.

make sure he is sitting as upright as possible and that the hole in the nipple isn't too big. Babies shouldn't belying to far down hen fed. They need to be sitting pretty upright, almost as if he were sitting up in a recliner. Maybe try a whole different kind of bottle all together. Has he always been like this or is it something he just started doing. He may even have a soar throat and doesn't want to swallow. he sucks and doesn't swallow and the milk squirts out. Or even an ear infection will make him not want to swallow.

He could have acid reflux and if that is the case a thicker formula. It could be a allergy to milk or lactose and might need a formula change. It could just be a premature digestive system due to being a preemie at birth. I would ask his pediatrician.

There are many different nipples for babies. YOU might have to try each brand/type to find the RIGHT one that FITS your baby. I always used NUK brand, But that was YEARS ago.
If you are still experiencing trouble CALL your doctors office and ASK them for their suggestions.
He might have something wrong with his suck/swallow or something else might be going on.
Changing the way you hold him might help also. Keeping his head more elevated could do the trick.
Was he ever breast fed. & how well did that go?

get him a speech theripist. they help with things like swallowing liquids and things of that sort. it is real important to get him gain weight since he was already a premi and smaller. i would also try a different nipple/bottle. this could be the reason. but seriously talk to ur dr about getting a speech theripist

try changing to a slower bottle nipple it may just be coming out to fast for him they also have one way nipples and the formula only comes out when the baby is sucking onthe bottle

Try a different type of bottle, also, there are different nipples for newborns so make sure to get one of those when you purchase bottles.
