if you are using a real hardwood you can just nail to the sheeting.if you are using laminat you just roll out a foam barrier and glue the flooring to gather and secure the edges. the sheeting in either case should be at least 3/4 inch.

Laminate flooring is 3/8" of an inch.

The minimum thickness for plywood is 5/8". 3/4" is recommended. Don't use OSB or particle board. Tongue and groove plywood is the way to go. Make sure your joists have good bridging installed.

Check with your local building inspector. There is a minimum thickness for subfloors by law. I think half inch osb should be sufficient. (oriented strand board or plywood)

your underlayment should always be at least 5/8" thick

it will vary from area to area, I'd recommend checking with the building inspecter
