I'd say technological advances, mainly long-line fishing and using big nets, are the leading cause. The problem with these methods is there is a whole ton of bycatch (literally, tons) that isn't the target species that is just killed. A large scale conservative fishing industry that actually only fished to its targets would probably be a lot less damaging than a smaller fishing industry that hauled in half bycatch along with it's market product.
