I know this sounds corny, but get a little planner and write down everything you need to do that night. It really helps you to stay on task and organized. Also, I don't know how your teacher feels about extra credit, but I would definitely try that. And ya know, maybe just allow yourself some study time every other day or something.

There are many things you can do, the important thing is not to get stressed out or start getting into the habit of procrastinating. Ask your teacher for extra credit, this helps bump up grades a lot. You could also get a private tutor, although this is not the best idea because the are generally expensive. A planner or a calendar in which you write everything down in a structured manner is helpful also. Remember--as Einstein said--"the important thing is not to stop questioning" Don't forget to ask if you don't understand something. And don't think you are alone in this kind of problem, there are thousands just like you all over the world.

I'd get organized. Ask your teacher what you can do to be organized. Ask her to be more specific. Get a tutor. I'd start worrying about it. Learn to time manage your classes and homework. Dedicate a certain amount of time each night to your math class, instead of just when homework is due. Find out what parts you are having difficulty in, ask questions in class. Some teachers when they say get organized they mean to get a notebook and folder dedicated to that subject, keep everything you do in it to reference back if needed. If you miss an answer in homework and the teacher doesn't go over it, stay after class or after school and ask her to go over it with you. Ask your guidance counselor, that's what they are there for.

My wife is a high school math teacher.
Spend some time with your math teacher after school a couple days a week for about 3 weeks. In that time they will definitely see that your study method can be improved and offer ways that will work with your personality.
A word of caution here: none of them include not doing the work.

Please ask your parents to hire a tutor for you. They can give you strategies to get organized and incorporate better study skills.
