Brian is the only one who can understand Stewie in detail. My impression is that when he talks to the rest of the family they perceive it as baby talk but brian hears him as an adult

i think only brian can understand him. like sometimes when stewie is talking to peter he just completely ignores it.

I think it's meant to be ambiguous (except for Brian). That's why they refer back to that question a lot, but never really give a straight answer.

I personally am not sure, it seems that most of the other characters interact with him perfectly fine. And hey, they have a talking martini drinking dog, whats so weird about a talking baby?
