Most probably the Queen. If you notice on even forgein tv she is refered to the Queen & not just the Queen of England for example. Whoever is US president (currently Bush) would be a close 2nd.

Tom Cruise: The Chinese love him (1.3 billion people) and to this day scream "SHOW ME THE MONEY!", Americans all know who he is (for better or worse), in fact, most of North and South America as well as Asia know him somehow. Africans also know him because of his part in the fight against AIDS when he was involved in it years ago.
HOWEVER, George W. Bush may also be the most famous/infamous person. There are few people worldwide who aren't familiar with him, and whats more, most have an opinion of some sort about him. He is creating a legacy of war and global unrest that will surely last longer than the rest of his term, so perhaps the President of the USA right now is the most famous person. Certainly, he is the most famous politician.

Madonna i think is recognised in probably every country. But in my opinion the most famous person dead or alive would have to be Elvis, although is he dead - and that is not a question i am going to ask here.
