They don't specify in the movie - they just throw you into the world where women can no longer have babies. Interestingly, in the book, it was men who were no longer fertile, and the first baby born is male.
Throughout the film, there is a lot of pollution and waste around, seeming to suggest the world has been poisoned. This, to me, felt like a good reason why women became infertile. But there doesn't need to be a simple cause and effect explanation - the movie is about so much more than that.

I really loved this movie by the way. Well, they don't tell you why women can't have babies because they don't really know why. Knowing the reason why wouldn't make much of a difference. The directing of this film was awesome and I highly recommend it to everyone.

They dont know what happened but think it has something to do with a form of chemical warfare causing infertility.
