I want to ask my Boss for a referral, but need to know what should be on one to have the best results.

Your boss should know what to put on it. When a prospective employer calls for a referral. They do not expect too much from the last employers. That's because he probably is aware that the old employers have to be careful about what they say about you.
If they bad mouth you in any way, they are damaging your character, and who's to say they are right? It is mostly the opinion your last employers have of you. You could sue some one for slandering and ruining your chances of a better opportunity.They most likely will just ask if you would be eligible for re-hire. If you are then that says something good about you.
