Her father is already married and has two sons, (from different mothers). He has not been a part of her life since i moved to a different city to go to school(about 2 years), but now that im back, hes not interested in seeing her either. I dont get child support nor do i want/need it. He moved to another city, but wont call or try to get a hold of her. I want to change her last name before she starts pre-kinder, because she keeps asking why she is the only one who has a different last name, and wont accept her fathers last name. Now i dont want him to be a part of her life anymore, besides she saw him about a month ago and didnt even recognize him. Hes obviously not a good father since he has 3 kids with 3 different mothers and only sees the one who's mother he's with, he doesnt have a stable job, has a criminal background involving family violence, and uses drugs. I just dont want him to have to do anything with my daughter at all.

Check with your local court house.
There maybe a petion or form that you can fill out and have him sign taking all of his parental rights away. It sounds like he'll sign if he doesn't have to pay support. The forms or other paper work will or should let you in front of a judge, change her last name.

Good luck and good riddens of a low life father...........can't even callhim that! It's his loss of his daughters life.........

I just got my name legally changed, although I am older, but it CAN be done. You will have to get her Father's consent, since she is minor, but if he wants nothing to do with her, most likely he will agree. I did all the legwork myself, and it only costed me $50 to get it done. In your case, $50 well spent!

you need to go to your local court office and ask for papers to change your daughter's last name.
