There is no such thing as diplomatic courier service, unless you are a foreign government or a diplomat. It sounds like you are being scammed (maybe you got an email saying you won the lottery?) -- if you can provide more information about the form you are filling out, I can give you better advice.

Answer nothing. This is a scam to steal your identity. Look at the questions..........do they tell all about you? I know, they promised to send you a lot of money, if you fill this out. They will NOT send you a lot of money. Did you get a check for about, just under $3000 from them? Are you supposed to send MOST of that back to them? Their check won't go through, it is fake. When it does not go through, and you have paid what you are supposed to pay them, the money will come directly out of your account.........your REAL money will be paid to them! If you fill out the form, you will give them all they need to steal your identity. I know it would be wonderful, but, it just isn't going to happen that you get all this money that they promise you. They are after information on YOU< and YOU are hoping so hard that the deal is for real. It isn't. Mega 2000? Do NOT give them ANY information on the phone, or on paper. You will get I.D, theft and it WILL COST you money if you follow their instructions.

You aren't doing this, by any chance, in response to a too-good-to-be-true offer from Nigeria?

If a stranger is asking your aid in moving funds from their country into yours, you should run screaming as quickly as possible. You'll give them your bank account information ostensibly for the purpose of transferring funds in -- and in fact, you'll unhappily discover that whatever funds you had in the account have been mysteriously transferred out.

Just don't do it.
