well, first you have the figure out the slope of the line:
slope (or m) =y2-y2/x2-x1
plug in the numbers for m=12-5/4-3
figure it out, m= 7/1 or 7.
since the equation for slope intercept form is y=mx+b, then plug in what you know which is m.
y=7x+b
since b is the y-intercept you would then graph the line to see where it intercepts.
in this case it would be -16, I think.
so, the final equation is y=7x-16.
