I have had it aerated. Now what should I top it with? I read to spread a layer of topsoil mixed with seed. Should I also use some sand, as the soil is quite heavy?

I would aerate your lawn, then using a mixture of sand and compost mix, top dress the lawn adding about 1/2" of mix to top of lawn. I am not sure that the seed will be necessary.

Apply a fertilizer high in nitrogen. If that doesn't remedy the problem perhaps you have a problem with pests. Generally a fading lawn is usually a good indication that it is requiring a boost of nitrogen. Hope this helps.
