Sounds feasible. There's enough I hate America to go around, enough American-built hospitals to keep an insulin dependent diabetic alive. And no U.S. embassy to rat him out if you did want the 25mil.

On the bright side his condition puts his him in a life threatening condition, especially when observing Ramadan. No food or drink all day long puts a diabetic in a very susceptible state.

Since he's probably being supported either by his wealthy family members or from Islamic fundamentalist charities, he's probably in a cozy, underground bunker in Pakistan, on the Afghan border. His communications are limited, but he knows what is going on, based on his released videos. He's hiding in a system of caves, most likely moving every day or every other day. (That's my opinion)
