Do you get approval from the girl you like to be her boyfriend first?
Or do you not mention a thing and just hold her hand and make those non verbal actions and poof! the next day you guys become a couple.
...without saying a word about a relationship...? Does it always happen like this?

you can hold a girls hand & such, but it doesn't make you a couple. it's always best to communicate to avoid misunderstandings, esp about the status of the relationship.

Actually no...it doesn't happen like that at all. You do not get any approval first, not just hold her hand as you'd probably freak her out and she'll end up automatically rejecting you...not because she doesn't like you but due to the embarrassment caused.

If you like a girl, the first thing that you should do is to seek to have some casual conversations with her. Get to know her a bit. Seek to show her kindness but do not overdo it as again, you might freak her out. Then, when the right moment comes, ask her if she would like to have a coke with you at a social public place where both of you are comfortable and relaxed. Don't suggest a movie, as she might think you're up to something...

You become a couple by time...there are loads of things to consider before actually becoming one...you might find out that after all, she might not be your type. Only time will tell.

Best of luck!!!

I think either way - if you slowly start holding hands and kissing, it's probably safe to assume you are a couple after a few days. That way, it may seem more natural than asking the other person and then proceeding from there, although that is fine, too!

If you go the nonverbal way, your partner will surely let you know, in a verbal way, whether or not he/she agrees!

Umm...not usually. There has to be some sort of mutual acknowledgment there.
