As gently as possible and in privacy. You might start with expressing concern because you are worried she might be ill, then saying you are noticing an unusual odor (avoid "foul" if you can) and want her to be healthy.

If it's too embarassing for you, write it as a note and give it to her discreetly.

She may be a little embarassed but if you are really friends she will eventually appreciate the honesty and the concern.

I would take her aside and gently let her know that you noticed this and to pls see her dr because you are concerned that she is ill as the other poster said.
