Why do they show people driving in films with both hands occupied doing something else the entire time?

Do the directors and editors realize or care that it looks fake?

It's one of those details that separates a good film maker from a a not so good one. It could also be that was the best take they had of the scene and the felt it wasn't worth the reshoot to fix that detail.

Have you also noticed how many times the characters are "driving" and it is obviously a green screen or a rolling screen behind them.
