While presenting an award for an Oscars, he translated Italian (not sure if it was Italian or Spanish).
Does Clint Eastwood speak Italian or was he being told of what to say by a teleprompter?

I think that he had a ear piece in because he could barley read the teleprompter without his glasses.

I'm Italian and I'm sure he was translating. The translation was not word by word but adapted to English what was the sense of the speech in Italian.
I know he lived in Italy at the time of his films with Sergio Leone
He also said some words in Italian in the same Oscar's night when he introduced the "clip" prepared by Tornatore for the Best Movie in a Foreign Language

Considering that he had trouble in the early years getting good parts in American films and that he spent most of five years in Italy making those 'spaghetti westerns' it would be safe to say that he speaks pretty good Italian.
