Language and content. But these days a movie like that would barely rate a PG-13. Movie ratings are not retro-active. More and more is permissible in PG and PG-13 movies these days due to the declining moral code in America. Not trying to be preachy and I love my country but facts are facts.

Prior to about 1998, the MPAA didn't publish specific reasons for ratings. I don't know if there is a specific database anywhere where you can find more information about the content of older films. But you should also remember that prior to 1985 there was no PG-13 rating so there was a huge valley between what was a PG and what was an R. So some films with an R rating would have actually gotten a PG-13 and some movies with a PG would have gotten a PG-13 (American Graffiti is one that comes to mind).

It was pretty racy stuff when it came out, we didn't have gang banger rap artists enlightening us on the seedy side of life then. One of the best movies ever produced as far as I'm concerned. If they still made them like that I'd be more inclined to spend my money at the movie theater.

There's an F-bomb fest, a two prostitutes, an insane guy that loses his virginity to said prostitutes, it's about crazy people, one of the patients mercy kills another...

It's a great movie, though.

Most likely b/c of its heavy content and the fact that it came out in 1975.
