Genetically not much so far as we know. Mentally quite a bit.
Once you get into this one begins to appreciate how rich in diversity the pantheon of humanity really is.
A shemale is a person in transition from one sex to another. It takes a bit of time and is dependent on hormone enhancements and surgical alterations. This costs and takes time.
One of the significant things that some medical jurisdictions do is request that these people, who are genuinely not comfortable with the sex they are born with, undergo a period of time, often six months, dressed in the clothes of the opposite but target sex.
In short the she-male is learning to be a she. And while it is enormously difficult for the person in that transition, they have to still learn female behavior. Reactions to surrounding events, she-males attempt a female behavior but often are either over the top or revert to a learned male reaction.
This drives those around them crazy to distraction and first impressions from strangers resulting in stifled giggles. And often their belief in what the appropriate female response is over the top. It is a like a amateur actor trying to do Hamlet, the best shot is comical and over acted.
She-males need that preparation period where the body is male but the clothes are female. Hormone effects can be reversed but not surgical alterations. Surgeries are final. That is the reason that the she-male tends to stand out. It is a faux existence for about a year before surgery.
Once the transition takes place the change can be dramatic and the medical skill superb. You know the old person going on about "Oh yes thats really a guy because they have an Adams apple."
Well I believe they even have a surgery for that too. Some surgeries are so effective that you really can't tell that this woman was once a man. I almost dated one without realizing it. Too bad a friend of mine got there first and was so happy. He found out later. It was a small town and there is no toleration for transition people. And less for them who can't tell the difference.
I haven't had the opportunity for a serious relationship with a trans-gendered person. I might. It depends on the person. Definitely the full transition must have been completed. It would be the same as dating a teen age girl. The horror. On the other hand. A babe but no babies.
Now she-males are different than transvestites which can be easily confused with she-males. It is just so easy to confuse the two. Transvestites or cross dressers aren't necessarily gay. Nor are they she-males. Many transvestites tend to be males who like to wear women's clothes but consider themselves male in every other respects. Although their fashion taste can be quite the quiche.
And it is very easy for the person who meets the female clad person for the first time in a social environment to be thoroughly confused. Its hard for the peson in transition.
The other confusion emerges when meeting an effeminate male who the gay community calls Twinks or Queens depending on the depth of the affectations, depth of make up and quantity of bad jewelery worn. These are males who act like women but are males who like other males who think that by acting like women they are attractive to other guys. Great I am getting a real headache. Skull side.
Finally back to she-males. The best way to think of the male-female difference is like, you're on a journey from Washington to New York. Well a she-male is Philadelphia.

A man knows that he is a man and takes great comfort, and even pride in it. A "shemale", a very offensive term by the way, refers to those in the porn industry. There are pre-op and non-op transsexuals who you may think are shemales, as they have breasts and male genitals, but they take no comfort in being thought of or referred to as men. Shemale is a term that has been adopted by the pornography industry and now refers to porn actors who have male genitals and female secondary sex characterictis, typically by implants and lots of surgeries. Obviously much of the things you see and look at has been digitally enhanced, as taking estrogen won't result in a flawless female body, nor allow one to become erect easily or have much, if any sperm after a few months.
