Because there is a little liquid in the bottom of the cup which creates an airtight seal around the bottom of the ice cube. This creates a suction/vacuum that makes the cube seems like it is stuck.

Because you put the ice in the glass first. When the water (or whatever) hit it , the ice bonded to the bottom of the glass.

Never thought about it but I guess you're right. I can't eat the ice...it hurts my teeth. They just be so sensitive.
