Most cancer in men will be a lump on the testical. (genital)

vasectomy areas can become tender, like mentioned above.

Probably he has epididymitis. Where the spermatcord goes into his testical. It can be caused by straining or an infection. Possibly it could be a spermatocyst, or varicocele. All of these are not to serious most of the time.

I would definitely take him to a urologist. (Even if it just for piece of mind.)

Just make sure that he is not having this medical emergency:
Torsion of testes - the testes hang in the scrotum on a cord which contains of vas deferens and blood vessels. It is possible for this cord to twist and cut off the blood supply to the testis . This is considered a medical emergency and requires an operation within a few hours of the onset of pain. The symptoms can be severe pain, then swelling and redness of the scrotal area. This condition requires emergency surgery to untwist the cord. Pain in the testis should be checked out by a doctor promptly.

It's most likely something other than scar tissue. Maybe a hydrocell or something like that. I had the same thing happen to me and I went to have it checked for cancer but luckly for me it was only a hydrocell and it was removed on outpatent babsis.

Well, if it's scar tissue it wouldn't be painful. Could be a testicular lesion..
