Well there could be multiple things your doing wrong.

Popping up:
-dropping shoulder (keep elbow up)
-opening up left shoulder to early (if your a righty

Chopping into ground:
-swing is on a slight down path (put bat on upper butt and wrap arms around and rotate to get proper level swing)
-WATCH BALL OUT OF THE PITCHERS HANDS!!!! (got me in a huge slump last year)

other things:
-make sure to rotate upper and lower body
-when turning back foot make sure other foot is turn at pitcher and stiff like: l\ (what you should look like with a little bend)
-swing hands at ball
-take advantage of power and rotate and use lower body

Get a batting tee; get a book on hitting by Charley Lau...then work on the mechanics. Big league hitters use a tee daily to keep their mechanics sound then all they have to worry about is reading the pitch and reacting, not how to stand or grip or anything else. All you need with a tee is a fence to hit against..or hang a tarp or blanket; a ball and a bat. It is any excellent tool and Lau explains the mechanics of a good swing well.

You can probably take lessons somewhere..But a good thing is to have 3 V's your arms should show 3 V's if you know what I mean. Make sure to have your head over your back knee. Swing- throw your hands at the ball directly at it. Your front foot should be at about 2 or 1 o'clock when you step toward the pitcher if your right handed.

you need to get a book or video to find out (1) how to correctly swing and (2) what is wrong with your current swing.
geting a private tutor for a couple sessions or going to a baseball camp is a great idea too.
then like everythign else, practice, practice, practice (hitting off a tee, taking swings, going to the batting cage)

http://www.tom-emanski-baseball-videos.com/

these videos help develop the correct swing path, bat slot, hip rotation and also give great drills. Especially for the young player not influenced by years of bad technique, but if you are older they will transform your swing completely. However, you have to give yourself to the changes and not try to keep parts of your old swing. If you really want to develop a powerful, line-drive swing then these videos are the right choice.

here are some sources for you to look up.
Book: The Science of Hitting by Ted Williams. Charley Lau was also a good hitting coach who wrote a book, but I do not remember the title.
Videos by Dusty Baker and Jim Lefebvre are also good.
