You can start off by saying
Dear... ( name)
I'm truly sorry for physically puting my hands on you,and I was very immature. I feel bad about what I did. I wanna change my act, and explain to you, that something came apon me at the moment. Where at an age now where we have to think twice about situations like this and start over again fresh. So maybe me and you can be friends, and not let this happen anymore. Well, bye!!!
Sincerely...

Whatever you write, be sure that it is something that you will be able to tolerate the entire class/school reading.
The person you fought with might not be as "BIG" of a person as you are and might not be at a place where he or she can forgive you just yet (especially if you 'won' the fight')
was it a physical fight or was it verbal.
Are you mandated to write this note or is it something that you WANT to do out of the kindness of your heart.
If it is something you want to do, I commend you. If not, I still commend you for the thought that you are giving it.
You might want to say that you are sorry for not finding another solution to your differences and ask if the other person will forgive you. I wouldnt personally say that, but it would be a nice touch if you are mandated to write this note.
I would not go into a whole bunch of details or blame the other person (or blame yourself)...I would keep it short and simple and ask if that other person would like to talk to you so that you can apologize in person (that way whatever is said will stay between you and that person and the note wont be any big deal if it does get into enemy hands). Also if it was a physical fight that other child's parents might want to have you suspended if it happened at school. No sense in giving them any free ammo....
Basically, my point is just be honest but be mindful of other readers.
Something like this would be my suggestion:
Dear (name),
I would like to apologize for my part in what happened between us. If you would like to discuss it further please let me know as I did want to give you a more personal and traditional apology face to face, however, if you are not agreeable to that, I do understand and hope that this will do.
sincerely, (your name)
END IT HERE!
Anything else that needs to be said can and should be said in person!
Best of luck!

Dear (name)
I am really sorry for the fight I had with you. I am really looking forward to be your friend, I didn't mean what I said. You are a really great person who has a big heart and I was stupid enough to say those things, I really hope we can come to an understanding and settle our differences and become friends. do you forgive me?
Ps: (i am really sorry you can put a sad face with tears)
your truly
