It may be as simple as someone who lacks attention and wants to be told that s/he is not fat or as complex as a Body Dismorphioc Disorder that may lead to other disorders such as Bulemia or Anorexia Nervosa.
Good news is, most women and some men just really want to be told that they're sexy and/or not fat. But if it reaches a point where in s/he is so bothered about being fat, then there's a problem and s/he should go get help.
