Depends on the context of the sentence:

The doctor told his patient's little boy to sit quietly.
The doctor told both patients to wait in the waiting room.
Both patients' kids were being noisy in the room.

its patients. meaning many patients. If you were talking about 1 patient you would say the patient's (patient is) going to be late. And patients' would be used in a statement of his or her possession or belonging. Like when you are referring to one patient and her medical history you will say the patients' file has been misplaced.

The first one is the plural form of patient which means more than one of course.
The second one is also plural, but it has an apostrophe s. In the regular way it would be patients's but it is not commonly used
