permit question?

I had my permit in my pants pocket and my mom put them in the washing machine. To go to the DMV what do i need? I have my permit reciept from the Driver's Education teacher, money to pay for a new one, and a photo I.D. Is there anything else I need? Does a parent have to go with me for me to get it or not because I have proof that I had my permit.

(I live in Illinois)

They have it on record, just take what you have and get a replacement and as my mom says.... I'm doing laundry check your pockets...
