How would I go about getting a medical certificate?

I need a medical certificate to return to class and explain to my professors that I have been sick. What is a medical certificate and what does it say? Do health clinics provide medical certificates?

It is simply a note saying giving the dates of your illness and the date you are able to return to activities (school)
You have to go to the doctors or clinics to get a one.

a note from the doctor stating that should be enough...if you were in the hospital a copy of your discharge papers might have the dates you were there
