Could someone please give me a simple example of a POA form for temp child custody? My husband and I are going on vacation and the kids are staying here. We want their grandma to have POA while we are gone and have her able to have them treated mediacally or anything else that comes up that needs parent or guardian consent. HELP!!

Nothing involving child care is simple, and you are crazy not to consult a lawyer in your state.

I do not recommend free ones, but you can find some online and may also be available in your local bookstore (Borders, Barnes & Noble) or places that have legal forms (Staples, Office Depot, etc.). If there is a legal library at your local courthouse, it may be open to the public and the staff can help you find a form.
