It depends on how low the charge is before you start charging again. If the battery is low on charge when you start charging...okay thats good...but...if you are hooking it up to charge on a regular basis and you still have two or more bars....not good. It is healthy for the battery to be drained almost completely and then charged up...not necessarily every time but in general, this is the rule.
As far as the amount of time spent on the charger...if it is a newer phone it should kick off the incoming current when the battery charge is complete without harm.
Take your phone off the charger at full charge...then keep your phone off the charger and see how long it lasts...with normal use
it should last a day and a half to two days before needing a charge...remember batterys have a life span...there will come a time when a replacement battery is needed.
My experience is that when a battery is charged as frequent as yours has been...It pretty much kills the life of the battery and its performance.
One more thing..when and if you get a new battery...let it charge complete before first use.

You really shouldn't. It can stress the battery to overcharge it. Let it lose most of its charge before plugging it in again, and then only charge it when it needs it. It should NOT need it every night, unless you're talking non-stop all day.

I dont believe it does. Though to keep the life of your battery, it's advised to let the battery drain low before recharging rather then recharging every time it loses a bar or two.

it depends on the phone. it will say on the manual whether is would be harmful on the phone or not. my motorola for example indicated on the manual that leaving the phone plugged even after it has fully charged wll not in any way harm my phone.
