Yes. Cheating is probably your most common answer. But I am going to be different and play devil's advocate. You can cheat, and have it be forgiven, and even sometimes forgotten. Not usually, but it does happen. What can't be forgiven or forgotten is any constant barage of disrespectful actions or comments or just general abuse. Physical abuse can heal, usually. But when you cut down on a person's self esteem every single day, without relent, it can take it's toll on someone so bad, that they lose their identity. And that is both unforgivable and unforgetable.

Any thing can be forgiven IF the person doing the wrong admits to it, apologizes and promises not to do it again and then sincerely asks for your forgiveness.

infedelity is never forgotten and even though they often stay together, they usually are never really forgiven. The pain stays and we never forget. Escpecially if a child comes out of it. not forgivable

it's unforgivable to not give it your all and try to make it work, especially if you have kids.

Never forgotten your wedding day, your first kiss, date, etc.

Yes, yes, yes. Especially when you're mad at your spouse. Every hurtful thing he/she has done gets blurted out and flung around the room like a giant ball of fire!!
