Check out Cafepress.com and set up a shop. They will handle everything about making the shirt once you upload your design.
That is just one option.

All small towns have t-shirt printing shops, if it is a printed design then you would need to get a batch made up by them to see if they really do sell as well as you hope they will. (Otherwise you will have nothing in your wardrobe but 10,000 of the same T-shirt! If you are thinking of a whole new take on the t-shirt design then I guess it's a manufacturer that you are after.
