We don't.

Many countries' head of state and/or head of government is someone other than a president. Often, it's the prime minister, but there are several kings and sultans left in the world as well. In several countries, the role is split, as in England, where the head of government is the Prime Minister (Tony Blair), and the head of state is the Queen (Elizabeth II).

"We" don't. Only republics have presidents. The rest have other heads of state (monarchs, Emperors, Emirs, Dukes, Chairmen, etc).
