In most cases for holiday visas it will require you to fill a form in and send off with your passport with fee to the Japanese Embassy in your country and in other cases you will have to travel to the Japanese Embassy in your country with your passport. For working visas it is a little more complicated - if you are thinking of staying in Japan after a holiday then you can change your visa once in Japan - this of course depends on many elements including what nationality you are.

You need to go to Japan Embassy. Fill up an application form and attached supporting documents representing that you can afford to stay in Japan. The easiest to get is a tourist visa but you need to show that you are financially capable like bank statement and other properties. Need your birth certificate, booking certificate, ID pictures (not passport size-they have their own measurement). Much better if you have friends or relatives that lives there who can vouch or guarantee your stay there. They need to send guarantee letter for you to attached also on your application.
