Coming from a klutz... I have sprained my ankle more time than I can count.
Generally the signs are swelling, the worse the sprain the worse the swelling - at one point in time it looked like a softball was coming out of my ankle.
Also bruising, and sometimes numbness.
My sprains have occured in various parts of the foot. I have had them on the inside and out side of my foot. Most of the time they are on the outside surrounding my ankle bone and around my heel. At one point in time the sprain and bruising went along the entire side of my foot and half way up my leg.

When a doctor or health professional is looking for a sprain, they are looking for a combination of symptoms. I am an athletic trainer and have seen more than my share of sprained ankles. When I assess an ankle sprain, I look for swelling, redness over the ankle, increased temperature over the area, and a loss of motion in the ankle. It is also important to know were the pain is located. The most common ankle sprain is over the outside of the ankle (lateral side) just in front or below the outside ankle bone (malleolus). This is where the ligament lies. A sprain is a stretch or tear of a ligament (tissue that connects bone to bone). This injury usually happens when we step on someone's ankle or step awkwardly and the ankle rolls inward. See the link below for more details.

A sprain will cause swelling and bruising, with pain. To rule out a fracture, an xray has to be done.
For treatment: elevation of the leg, ice and something for pain.
