Because I am a loser and have listened to the commentaries on the DVDs I can tell you exactly what Seth MacFarlane and company have said about this subject. Apparently the family can hear Stewie when it is good for the plotline. When it's not, everyone just ignores him.

There are a couple of episodes where she does understand him. But mostly Brian talks to him. I honestly think the majority of time they just feel it's baby babble. But then again most of the time that family is so wrapped up in what ever they are currently doing that they ignore Stewie anyway

I am a big fan of this show, have all the seasons on DVD, but I am not sure if anyone does understand him. There are some times when it seems like they do, but who knows. It doesn't matter Stewie is the best character followed by Quagmire.

I heard that like if stewie says " Screw You Lois!" Thats what you hear. But Lois hears " I hate you mommie!" Or if Stewie says, "Mother, Get me some dinner and I'll spare your life." But Lois hears, "Mommy, get me dinner or I won't love you anymore!" You understand what I'm saying? Whatever Stewie says, Lois translates into baby gibberish. I hope I helped.

I'm a big fan of that show too, but it's the age old question! There was even an episode (The episode when Peter formed Peteroria) and they did a flash forward to the future and were talking about Peteroria and a member of the class studying Peteroria asked "So like, can they understand the baby?" and the question went unanswered.....

good question unless Lois is just in denial that he hates her. But that would make since if only Brian understood him cuz he's a dog and dogs aren't suppose to talk so he can understand other gifted people who can talk but aren't suppose to
