That is a actually a very good question. A lot of times you'll see someone you maybe haven't seen in awhile, and you'll say "Hey, how've you been?" It's basically a greeting or an instinctual thing to say these days.
Or you'll ask your desk neighbor at work, "how are you?" Have many times do we actually end up asking that question on a daily basis?
Personally for me, usually when I ask, I genuinely want to know. If I don't want to know, I will just do my usual greeting..."good morning", or "Hi so and so".
I honestly do get tired of hearing that everyday from the same people. It's probably going to be the same answer I gave you yesterday...please move on! If your boss asked you or someone you barely tolerate, are you honestly going to tell them how you are? Especially if you don't like your job. Probably not.

today, my dad came home from a week away at work, i really miss him when he goes, so when i see him and ask how he is, i really do care, but you're right, most of the time it's just a reflex isn't it, you know, the great greeting "Hi! How you doing?" lol
