I'd have to say the Terry Gilliam film 'Brazil.'
I think that the title of 'Pulp Fiction' relates to the type of film/story that it is, rather, a pulpy sort of thing, like an old, dime novel. Just because a title doesn't directly relate to something IN the film doesn't mean it's irrelevent. Like 'Million Dollar Baby' means something, even though it isn't referenced in the film.

Magnolia.. the movie is about several different story-lines and characters whose lives are somewhat related, or mirror each other.

How about "The Crying Game" for a non-matcher? If you can relate that title to the surprising ending you are a true psychic.

Lake Placid. A movie about giant alligators in a lake, but the lake's name is not Lake Placid. Not sure why.

Maybe there's a reason why but I haven't noticed or found out why the title.
