Run an untested update or delete query without first backing up the original database. That's always good for a few laughs

Password protect the database and forget the password or findout that the VB front end nolonger works because it is not password enabled.

I worked in a treatment plant where the operators used walkie talkies. If the radio was transmitting near teh PC the Screen would turn to garbage. This was all well and good untill the transmitter was used during some critical write to the hard drive. There must have been enough EMI (electro magnetic interference) generated to scramble signals to the hard drive and not just the video card. Reinstalled the OS to fix that one, lost some current data.
