In a variety of manners, such as physiological reactions on the aural senses through raised blood pressure and awareness plus also mental self-organisational responses and 'Dream-State/Reverie' reactions ....etc.

There may also other more philosophical ways of considering this question as, for example, how do you know that you are thinking? What is hearing? etc

Fascinating question ......good one !

Sometimes. I've got to be thinking about something real intently and I get to a point where i get in a "zone" and can't hear anything else but my own thoughts. Doesn't happen very often though...lol

Yeah! I'm really verbal anyway so my inner voice is always prattling! I never think before I speak so my internal and external utterances are always totally spontanious so it can get a bit mad. I suffer from manic depression but I'm on medication now so it's not too bad but before if I was manic it was much much worse! I talk to myself too if I am concentrating on something but everyone in my family does that. I used to work in an office which was all women and we all used to talk to ourselves when we were working, it was hilarious cos no one ever knew who was talking to whom.
