Malnutrition arises when a person's diet does not include one or more of the required things.

A lack of intake/absorbtion of the essential vitamins, minerals, proteins, fats, and sugars in ones diet...

this can be caused by problems in ones diet (the most obvious and easiest to cure), or it can be caused with a problem in ones small intestine (maybe Crohn's disease, or a parasitic infection...sometimes the same problem).
