No, you are not required unless you want treatment. Most doctors will need your SS# in order to file the insurance. It's such a small thing, and so long as they promise security of their system, then you'd have nothing to worry about.

It is just like a bar, or restaurant, if you have to give SSN to place an order, thats the rule, owner has the right to refuse service to anyone.
I do not know if your State requires it, so I have no idea about that law. Ask an attorney, consultation is free.

Many facilities do this for two specific reasons, the least of which is to secure positive ID. It's to ensure they get paid. If insurance fails to pay or you have none, if you fail to pay your bill, having your ssn allows them to get a court order to garnish your wages.

In the military they file you by SSN but in a doctor's office they usually file you by last name. So why would they need it?

No, but if you have health insurance and they use your social as your policy #, then you may have to pay your own doctor bills since the doctor can't file without it.
